New Patient Appointment Policies
At Deep Rooted Chiropractic, we are honored to support families in building a strong foundation of lifelong health and wellness. To ensure each new patient receives the time, attention, and preparation they deserve, we kindly ask that you review the following policies before your first visit.
1. Appointment Requests and Confirmation
Please note that our online new patient form is solely used to request an appointment. Submitting this form does not confirm your visit.
A member of our team will personally reach out to you to review your request, answer any questions, and get your appointment officially scheduled. Only once that scheduling call is completed is your appointment confirmed.
2. Credit Card on File Requirement
To reserve your new patient appointment, we require a credit card to be placed securely on file at the time of scheduling.
Your card will not be charged when collected. This policy ensures that the time we’ve set aside for you remains available and respected. Your payment information is stored securely using HIPAA-compliant systems.
3. Completion of New Patient Paperwork
In order to provide personalized, focused care from day one, we require that all new patient paperwork be completed at least 24 hours before your scheduled appointment time.
This allows our doctors and New Patient Specialist to thoroughly review your health history and prepare for your visit. If paperwork is not completed within this window, your appointment will be automatically canceled and may need to be rescheduled.
4. Appointment Grace Period
If you are running late to your scheduled appointment—whether in person or a phone consultation—we provide a 15-minute grace period.
After 15 minutes with no communication, the appointment will be marked as a no-show and canceled. If you anticipate being late, please contact us as soon as possible so we can do our best to accommodate you.
5. Cancellation Policy
We respectfully ask that any cancellations be made with at least 24 hours’ notice. This allows us to offer your appointment time to another family in need of care.
Appointments canceled with less than 24 hours’ notice will incur a fee equal to 50% of the new patient visit cost. This charge reflects the time and preparation that has been dedicated to your visit.
6. No-Show Policy
If you miss your appointment without notifying us in advance, your card on file will be charged the full cost of the new patient appointment.
As a busy pediatric and prenatal office with a full schedule, this policy helps ensure that appointment times are reserved for families who are ready to begin care and committed to showing up for their health.
